Care Manager

Position Summary: The University at Buffalo’s Center of Excellence for Alzheimer’s Disease (CEAD) is seeking a highly qualified candidate for the position of Care Manager. One of ten Centers of Excellence across New York State, the WNY CEAD is part of a major public health initiative charged with improving the rates of early detection, diagnosis and improving care for people with cognitive impairment and their caregivers. Reporting to the Program Director, the Care Manager will, along with other related duties as assigned, provide services related to the goals of the Sponsor approved work plan including, but not limited to:
1) Providing direct patient and caregiver assessment, care management and follow up services at lead diagnostic sites across the seven county region.
2) Working together with other team members, the Care manager will train and educate health care professionals in Primary Care practices and other settings across the region on the development of dementia specific plans of care, screening, assessment, care management and /or other topics as appropriate to meet provider needs.
3) Actively engage in ongoing program evaluation activities, collaborate with colleagues at the Department of Health on new initiatives and support the interaction and collaboration of the three lead diagnostic sites to meet work plan goals and objectives as outlined in the approved work plan.
4) As a member of the team the Care Manager will promote and represent the Center of Excellence for Alzheimer’s Disease and its work across the region.
